Multi-Factor Authentication (MFA) for the Demetra Web Application

The ability to add Multi-Factor Authentication (MFA) for the web application has been added to the user settings. Users will, upon logging in to the Demetra Web Application, be urged to configure Multi-Factor Authentication (MFA), as in a later software release the usage of Multi-Factor Authentication (MFA) on the Demetra Web Application will become mandatory. Once MFA has been configured, when logging in to the Demetra Web Application, users will need to enter their username and password as well as a one-time password code from the selected authenticator app.

Quicker log out from Demetra scope

When the Demetra scope goes into standby, by default, the user will be logged out after 15 minutes. This timescale can be adjusted on the device by changing the ‘Device Settings’ in the ‘Settings' menu’.

Automatic save of last captured image

If a user has captured an image and on the condition a patient has been selected, but forgot to save or delete the image, prior to being logged out of the Demetra scope, the user will receive a reminder that there is still unsaved data. The message “save & log out” or “delete and log out” will now appear.
If a user’s device goes into standby but is not logged out, a pop-up message will still appear as before, informing the user of unsaved data, providing the option to “end consultation” or “continue”. This scenario is also on the condition a patient has been selected.
Note that the image cannot be saved if no patient has been selected, resulting in the loss of the data.

Accelerated workflow available for existing overview image

When the user selects an existing overview image from the patient gallery, they will be able to add multiple markers or select existing markers, to capture multiple lesions. With this extension, the user can now benefit from an accelerated workflow for (capturing) multiple images, either when starting from the Overview mode on live camera or from an existing overview image on the patient gallery.

Possibility to access collaboration request of other users within the account

For accounts with multiple users who have access to all patient records, users can now also view, download, and respond to collaboration requests that are shared by or with other users of the account. When visiting tab ‘Collaborations’, 2 additional tabs will be visible: ‘Shared with account’ and ‘Shared by account’. The notification-mechanisms to inform of newly received or newly responded collaboration requests, now also reflect any actions other users within the account have done.

Status-filter for collaboration requests

On the tab ‘Collaborations’ users have the possibility to filter on “All”, “Unanswered” or “Answered”. All 4 collaboration tabs (“Shared with/by you” & “Shared with/by account”) now have totals, based on the tab- and filter-selection.
The Left number on the total (left number/right number) is adjusted according to the filter, this has a maximum of 1000, as it is based on the number of created requests that can be viewed on the page.
The Right number on the total (left number/right number) is unchanged by the filter-selection, can be more than 1000, as it is based on the number of sent requests in the database.
Note that if users saved a draft-request the left figure can be higher than the right figure, when filter ‘All’ is selected.

All images of all consultations are displayed in the patient gallery on the device

Users can now consult all images of all consultations on their device and not only the most recent image from the previous consultation of any given lesion.

Accelerated workflow for capturing multiple images

When starting from the Overview image mode on the live camera, users can now capture images of multiple body parts and multiple lesions in one go, via an intuitive workflow.

Additional features on consultation report

The consultation report was made more customizable. Users can now select which images of the selected consultation date they want to include in a consultation report, in addition to adding a personalized header and signature to the PDF report as well as adapting the report title. Except for the adaptation of the report title, these options were already available for the collaboration reports since the previous release.
Upon creating a new consultation report, the user has the option to add relevant details in the ‘Header’ and ‘Signature’ fields on the ‘Information’ tab. In case header and/or signature information was already provided for an earlier consultation or collaboration report, this info will be prefilled. The personalized header will appear at the top of the first page of the PDF report, the personalized signature at the bottom of the last page, with a blank space to manually sign the report if desired. On this tab the user can also adapt the report title of the consultation report.
When proceeding to the ‘Content’ tab, users will be able to indicate which images of the selected consultation date they want to include in the consultation report. By default, all images of the consultation will be selected.

Adaptation on collaboration report

The images on a collaboration report will be visualized, both on the PDF report and on the interactive report, in descending order, with the most recent image at the top.

Gradual transition to mandatory and complex pin code

During onboarding, new users of an existing account will only have pin code as device authentication option. This will be the only authentication option visualized in the user settings.

EMA V2 integration

With EMA v2 integration enabled, the EMA patient database is the single source of patient data, and no patients can be created directly in Demetra. When you search for a patient in Demetra (either on the device or in the web application), you are searching directly in the EMA patient database. When the desired patient is retrieved (either on the device or in the web application), the patient demographics (Last name, First name, Date of birth, Gender, Patient ID) are taken over from EMA, and the patient is created in Demetra (or updated in case the patient already existed in Demetra). On the device, you can search for a patient from the EMA database using Last name, First name, Date of Birth or Medical Record Number (MRN). You can select the desired criterion by tapping the orange filter icon. After a consultation in Demetra, all images as well as a pdf report of the consultation are automatically synced back to the appropriate patient file in EMA.

Teledermatology

As a skin imaging platform, Demetra has been designed to support collaborative patient care and improve access to specialist dermatology care. Within Europe specifically, the intended use for Barco Demetra has now been extended so that primary care providers, nurses and pharmacists (the latter to the extent permitted by local legislation) can use Barco Demetra to capture dermoscopic images and clinical photographs and, by way of the collaboration functionality, share these with dermatology specialists to obtain remote clinical advice. By including both high quality dermoscopic images and clinical (overview) images in a teledermatology request, the reliability and accuracy of the clinical decisions or diagnoses made via teledermatology can be improved.

Additional features on collaboration report

The collaboration report, available in Europe, was made more customizable. Users can now select which images of the consultation they want to include in a collaboration report, in addition to adding a personalized header and signature to the PDF report. Upon creating a new collaboration request, the user has the option to add relevant details in the ‘Header text’ and ‘Signature text’ fields as part of the ‘Information’ tab. Users who currently do not use a pin code as device authentication, will, upon logging in to the Demetra Scope, be urged to set a pin code, as in a later software release the usage of a pin code on the Demetra Scope will become mandatory. Additionally, users whose existing pin code is not complex enough, will be requested to set a new code. A complex pin code does not consist of identical digits (e.g. 1111) nor of consecutive digits (e.g. 4321). A pin code can be selected or reset at any time via the user settings in the Demetra Web Application.

Tapping ‘AF’ button to enable manual focus when capturing images

When capturing a dermoscopic or close-image image, the manual focus will now only be enabled if the user taps the ‘AF’ button to disable autofocus. The focus can then be adjusted manually by scrolling the right touchpad on the grip of the Demetra Scope. The autofocus can be re-initiated by tapping the ‘AF’ button again on the live camera mode.

Faster capturing of multiple overview images

The new capturing functionality allows users of the Demetra Scope to capture multiple overview images more quickly by selecting multiple body parts at once. This new functionality can be initiated by selecting the ‘Overview’ image mode, which allows the user to select multiple body parts on the dummy mannequin. When all relevant body parts are selected, the user can proceed to the next step and capture overview images for the first selected body part. Upon tapping the ‘Next’ button, the captured image(s) are saved, and the user is directed to capture the second selected body part. This process is repeated for all selected body parts. The user can then add lesion marker(s) for the latest captured overview image. By selecting the relevant overview images in the gallery, lesion markers can also be added for overview images captured earlier in the flow.

Instructions for use accessible on Demetra Scope

The most recent instructions for use (of all applicable medical devices) are available on the Demetra Scope in the language(s) relevant to the account and user. The user can access the instructions for use via the settings menu by selecting Instructions for Use. As before, the most recent instructions for use also remain accessible in all available languages on https://www.barco.com/nl/support/demetra/docs.

Patient contact details

In addition to the existing patient information, users can now add notes and contact details to their patient records. In the Web Application, you can now optionally add notes, email address, phone number and address to a patient record at any time through the ‘Patient information’ page. On the Demetra Scope, you can now optionally add patient ID, email address, phone number and notes to a patient record when creating a new patient. These details are also included in clinical reports.

Image transfer

The Web Application now allows transferring unlocalized dermoscopic images and close-up images from one patient to a different patient within the account. Overview images can only be transferred if no lesions are localized on it.

Navigating through (filtered) images in the Web App

In the Web Application, the user can now loop trough the different images as selected in the image gallery, either by using the arrows left and right of an image, or by using the keyboard arrows. Any filters and sorting options applied in the gallery prior to opening an image detail page, are retained for the session or until the user clear them.

Sharing patient data (Collaboration) - EMEA

The new collaboration functionality, available in the EU market, allows users of the Demetra Web Application to securely share selected patient data stored in Demetra with other medical professionals. The patient data is intended to be shared for informational purposes only, not for diagnostic purposes. A collaboration request can be initiated in different ways:
  • Click the Collaboration tab in the top menu at the left hand side of the Demetra Web Application and select Requests shared by you. Click the button Create a new request, select a patient and confirm to create the request.
  • After having selected a patient, the Consultations tab is opened. Under the appropriate consultation date, click Collaboration request in the dropdown menu under Generate report.
  • After having selected a patient, click the Reports tab, then click the Collaboration requests subtab on the left hand side. Click the button Create a new request.
The recipient of a collaboration request, whether or not they are an existing Demetra user, will be notified by email. Once they’re logged in as a Demetra user or a guest user, they can either send back their comments, or download the collaboration report for their own record. Consult the User Guide for more details.

Option to hide the User Interface

In the Web App, it is now possible to remove the User Interface elements when comparing images in fullscreen mode, such that no text or buttons are displayed on the images anymore.

Option to include markers when downloading overview images

In the Web App, it is now possible to download overview images both with and without the markers that are used to localize lesions on that image. You can make the choice after clicking the 'Download' button for an overview image.

Data Export

You can now export the full data (including all images) from your account via the User Settings menu in the Web App. Consult the user guide for more details on how this works.

Optional race & ethnicity fields (US)

It is now possible to add race and ethnicity information in the Patient Information screen in the Web App.

Onboarding flow for 'other' users 

An onboarding flow was added for new ‘Other’ users (non-clinical users). In addition when you now ‘promote’ an Other user to Clinical user, they will now have to run through additional onboarding steps before they can use the web app as a Clinical user.

Patient Sorting

In the Web App, the patient list will be sorted by most recent 'Consultation date' by default (as opposed to by last name). Clicking the 'Last consultation' label again will reverse the order to show the empty consultation dates first.

Empty patient list

When EHR integration is enabled and the patient list is empty, the copy explains that patients can be added through the user’s EHR system. When EHR is not enabled and the patient list is empty, the copy explains that the user can add a patient by clicking the + button.

Secondary camera bug fix

Fixed the bug causing the secondary camera to fail when taking a follow-up (close-up or overview) image

Dermoscopic image limit bug fix

The user will now receive a message if the dermoscopic image limit is received (i.e. also in case of an error with the subscription) and the image will remain saved on the device until the dermoscopic image limit is increased (after which it will be uploaded to the cloud)

Image capture speed

The image capture speed of the Demetra Scope has improved:
  • The time needed for a complete multispectral dermoscopic image capture is reduced significantly. When waiting for the image capture sequence to be finalized, the two different waiting screens (one for capturing the images, one for processing the images) have been replaced by one waiting screen.
  • Capturing clinical overview images is much more instant.
  • The image capture for clinical close-up images is faster as well.

EHR integration

This software update allows to enable integration with an Electronic Health Records (EHR) system for dermatologists in the United States, i.e., EMA from Modernizing Medicine. This is a first step towards future integrations with a broader set of EHR systems both in the U.S. and in Europe. For more details on Demetra’s integration capabilities with EHR systems, please contact your Barco sales manager or partner.

Other improvements

  • The Demetra Web Application gives a better overview of your subscription and optional add-ons. For admin users, this information is shown in the ‘Users’ tab in the ‘Admin settings’. For regular users, this information is shown in the ‘Account information’ tab in the ‘User settings’.
  • Management of users by an administrator has been simplified. There are now two categories of users: ‘Clinical Users’ and ‘Other Users’. Clinical users have access to patient records, other users do not. Any user can be dragged from one category to the other in the ‘Users’ tab of the ‘Admin settings’ (in the Demetra Web Application).
  • In the patient image gallery in the Demetra Web Application, multiple filters can now be combined (diagnosis, management and image type).
  • In the patient image gallery on the Demetra Scope, when an image thumbnail is tapped, it will be downloaded from the cloud immediately (instead of being downloaded in a fixed sequence).

Improvements to the 'offline' mode

Over the summer, we released the offline mode, allowing you to keep using the Demetra Scope without an active and stable WiFi connection. With this update, we further improve this functionality, making it faster and more stable.

Image filters

In the Web Application, you can now filter the patient and account galleries by multiple image types at once.

Onboarding flow

Various improvements have been made to the onboarding flow for new users.

Privacy settings

Admin users can now update the privacy settings of the account via 'Account Settings' (Web Application). Next to this, every user has the possibility to grant / withdraw consent for linking performance feedback to their user identity through the 'User Settings' screen in the Web Application.

Demetra Scope: offline workflow

With the latest software version, the Demetra Scope will remain fully functional in case the WiFi connection on the device is (temporarily) lost. The supported offline workflow now includes:
  • Logging in with any user available prior to the WiFi connection being lost.
  • Starting a consultation for any patient available prior to the WiFi connection being lost or for any new patient created on the device. This includes taking new images, viewing existing images, taking follow-up images (with transparent overlay of the previous image), comparing images, and assigning or editing a diagnosis or management strategy.
The device memory will be synchronized to the Cloud as soon as the WiFi connection is restored.

Secondary camera image quality

  • The focus area of the secondary camera (clinical overview and close-up mode) can now be defined by simply tapping the screen, if desired.
  • Improvements have been made to the image quality of the secondary (clinical) camera (based on image processing).

Cloud region selection

When browsing to demetra.barco.com for the first time after the software update, you will be required to select the region where your account is hosted in the Cloud, being either Europe or the United States of America, before you can log in. If your login fails, please verify that the correct region was selected prior to authentication.

Clinical report generation

Clinical reports are now generated by Demetra Cloud instead of the Web Application. As a result, you will no longer be able to see a preview of the clinical reports in the Web Application before they are generated. Instead the pdf report can immediately be downloaded or opened in a new browser tab.

New functionalities and improvements

Improvements in the Demetra Web Application

  • Multi-select filtering is enabled for the Diagnosis and Management filters in the gallery.
  • Aside from last name, first name, and patient ID, patients can be searched for by date of birth
  • Updates to the Skin Parameter Map functionality:
    • In order to provide feedback, users can assign a star rating to every generated Skin Parameter Map.
    • In ‘Quick compare’ mode, zoom, pan and rotate are now locked by default for all compared images and may be unlocked through the toolbar at the bottom of the screen.
  • New users added as “Other users” (i.e. not linked to a subscription) are granted admin rights by default, so that they can proceed with the onboarding flow as soon as they are invited. Admin rights can still be removed for these users by unchecking the ‘Admin rights enabled’ checkbox.
  • On the “Users and licenses” screen, admin users can now drag and drop users between subscriptions and delete users from the overview screen, confirming changes through a pop-up. User profiles can be accessed via the eye icon.
  • The personal settings previously found on the “My profile” screen, can now be accessed through “User settings”, where a distinction is made between “Personal information” and “User settings”.
  • On the patient “Risk Factors” screen, it is no longer needed to indicate an exact number of atypical nevi. Rather, when “Yes” is selected”, one of the following options will have to be chosen: < 5 / 5 – 9 / 10 -25 / > 25 / Unknown.
  • Several bug fixes are also included.

Time zone based on region

The time zone for all devices in an account will be based on account country, and can be changed per device in the Demetra Web application via the “Device overview” screen under “Admin settings”. 

Localization of date, time and number format

The date, time and number format will now depend on the account country.
  • The default time format is the 24-hour time notation, but this may be adjusted to a 12-hour notation.
  • The date format will be set to MM/DD/YYYY by default for US and to DD/MM/YYYY for all other countries. Additionally, users have the option to set their date format to YYYY/MM/DD.
  • The default number format will include a decimal point for US, and a decimal comma for all other countries.
All format preferences can be updated on user level via the “User settings” in the Demetra Web Application.  

WiFi connection with security certificates (802.1x support)

Setting up a WiFi connection protected using WPA2 Enterprise and using EAP-TLS is now supported.
  • The required WiFi certificates (user certificates, CA certificates or both) can be uploaded to the cloud by admin users via the “Device overview” screen in the Demetra Web Application
  • The WiFi certificates are automatically downloaded to the Demetra scope and can be used in the network configuration after the scope has been onboarded via a temporary network.

Updated onboarding flow

  • While the default language for the onboarding flow was previously English, a preferred language will now need to be selected on the Welcome screen and will be applied immediately to the subsequent onboarding steps.
  • The role selection step is omitted from the admin onboarding flow, granting all admin users access to patient records by default. Access to patient records can still be removed for admin users in the “Users and licenses” screen.
  • The account color scheme can no longer be selected as part of the admin onboarding flow, but can be changed by admin users via the “Account settings” in the Demetra Web Application.
  • After finalizing the Web onboarding flow, admin users are redirected to the “Device overview” screen, where they can immediately add a device to the account.

Demetra Scope: Automatic log-out overnight

  • When there is no unsaved data, a user is automatically logged out overnight (between 12 am and 3 am) .
  • When there is unsaved data left on the device, a dialog is shown instead, where the user has the option to save or discard the unsaved data before being logged out.
  • The user is never logged out automatically when the device time is between 3 am and 12 am.

Login URL

The login URL for the Demetra Web Application has changed to https://auth.demetra.barco.com, replacing https://barco-healthcare-ads-prod.auth.eu-west-1.amazoncognito.com/. This is the URL of the page after clicking on “Go to Login” when visiting demetra.barco.com. In case you have saved the previous link in your web browser favourites, it will no longer work. We recommend that you instead save https://demetra.barco.com in your favourites.

Updated diagnosis list

The list of available (tentative / pathology-proven) diagnoses has been finetuned, in cooperation with our clinical partners. Especially note that all lesions previously marked with the “Other” diagnosis will now be shown as “Other malignant lesion”. This means that the diagnosis of benign lesions that were previously indicated as “Other” will need to be manually changed into “Other benign lesion”.
Below you can see the full list of diagnoses you might have entered before this software update, that will be mapped to a new diagnosis in this new software version:

Updated management strategies

The available management strategies have been updated:
  • If no management strategy is selected, “Select a strategy” will be displayed as default instead of “No follow-up or treatment”. A grey marker is associated with this state.
  • The “No follow-up or treatment” strategy is shown as a green instead of grey marker.
  • All grey markers previously indicated as “No follow-up or treatment” will keep their grey colour but will now show “Select a strategy”.
  • The blue management strategy is adjusted from “Treatment, excision or biopsy finished” to “Excision or biopsy finished”. 

Patient logout

In order to prevent saving images in the wrong patient record, the device will automatically return to the patient selection screen when it wakes from standby mode. The device enters in standby mode either by shortly pressing the on/off button or automatically after 10 minutes of inactivity. In case there is any unsaved data, the device will show a message. The user can then tap “Continue” to return to this patient’s data in order to save it, or tap “End consultation”, which will remove the unsaved data.

Keyboard configuration

The keyboard layout on the device (qwerty, azerty, qwertz or another derived layout) will now depend on the combination of the selected language and the (newly introduced) keyboard region (country code). 
  • When your language is set to “Dutch”, your keyboard region will automatically be set to “BE” (Belgium) during the software update, resulting in an “azerty” device keyboard.
  • When your language is set to “English”, your device keyboard will be “qwerty”, regardless of the selected keyboard region.
  • More details for other languages and other keyboard regions can be found below.
The keyboard region can be adjusted via the user’s “My profile” in the Demetra Web Application.

Other improvements on the Demetra Scope

  • The patient list can be sorted on last name or on consultation date. When patients are newly created via the Web Application and the patient list on the device is sorted on (last) consultation date, all new patients are shown on top of the patient list on the device.
  • Numerics and symbols can be used in the first name and last name when creating a patient. (This was already possible via the Web Application.)
  • After a new patient has been created on the device, this patient is automatically selected and the camera (live video) mode is opened.
  • When taking a follow-up image and then changing the image mode (from dermoscopic to close-up or vice versa), the transparent overlay of the previous image disappears.
  • The settings menu on the device displays the device software version.

Other improvements in the Demetra Web Application

  • In the Compare mode:
    • Each image can individually be reset to its original state after rotating, panning and/or zooming on the image, via the button at the top right corner of the image.
    • A whitelight dermoscopic image can be compared with its skin parameter map(s), after those have been generated.
  • In the Risk factors of the Patient information:
    • Multiple diagnoses can be selected per family relationship in the “Family history” field.
    • When “Yes” is selected for “Atypical nevi”, it is no longer mandatory to specify the exact number of atypical nevi.
  • When reviewing an image in the Web Application, the horizontal and vertical rulers that can be displayed on the image are extended to the full length and width of the image.
  • The Web Application includes an “About Demetra” page, accessible from the drop-down menu at the top right corner, which includes the medical device information (part numbers, software versions, legal info, link to user guides, etc.).
  • The Demetra Web Application shows a help button at the top right corner, which takes you to the Demetra Support page on http://www.barco.com. Check out the knowledge base for interesting FAQs.
  • A few updates were done related to accessing the Demetra Web Application:
    • When closing the web browser or web browser tab running the Web Application, the user will be logged out automatically.
    • Different users can be logged in to the Web Application in different browser tabs at the same time.
  • A few updates were done in the onboarding flow for new users:
    • The onboarding email that is sent automatically to every new user has received an update.
    • During the onboarding flow, the options for user authentication on the device (“Pin code” or “None”) are immediately visible, without having to click on a link.
    • The “Keyboard region” will need to be selected during the onboarding flow, which – in combination with the selected language – determines the keyboard layout on the device (qwerty, azerty, qwertz or another derived layout).

Demetra Scope

  • Fixed a number of issues that could lead to synchronization errors (server error) and images not being uploaded to the cloud. From now on, in case a synchronization error does take place, the user will see a pop-up message and is asked to contact the Barco helpdesk as soon as possible.
  • Log files of the device will be stored in the cloud, allowing Barco to investigate and solve problems when they take place
  • The menu to select an image type (dermoscopic, close-up or overview) will be opened by default, e.g. when selecting a new patient.
  • When capturing a dermoscopic image, there will be a clear split between the actual image capture (during which the device should not be moved) and the image processing step.
  • The default camera for taking a clinical close-up image will be the secondary camera instead of the primary one. It remains possible to change the camera.
  • The default mode for localizing an image will be on an overview image (instead of on the mannequin). It remains possible to switch to the mannequin mode by clicking the appropriate icon.
  • When taking a follow-up image of a dermoscopic or close-up image, by default the video preview of the same mode (dermoscopic or close-up) will be shown, including a transparant overlay of the previous image. It is possible to switch to the other mode, which allows to quickly take follow-up images in both dermoscopic and close-up mode.
  • The network settings screen now also includes a connectivity test which measures the upload and download speed of the network connection, indicating whether these are sufficient to allow a good performance of the device.
  • It is possible to connect to a WiFi network that requires a web (captive) portal
  • Various small improvements to the user interface have been made

Demetra Web Application

  • When the web application is not used, the user will be automatically logged out after 2 hours. As of 10 minutes before being logged out automatically, the user will be informed by means of a timer, shown in the web application as well as in the browser tab.
  • When a dermoscopic or close-up image has been localized on a clinical overview image, the web application will automatically show this overview image (instead of the mannequin). When an image has been localized on the mannequin, the mannequin will be shown. It remains possible to switch between both.
  • In the 'Consultations' tab, each consultation will now show the physician that captured the images, as well as the user that last changed the consultation.
  • In Admin Settings (under Account Settings) the color scheme and language of the account can be changed.
  • "My Profile" now contains a link to the product privacy statement of Barco.

General

  • When creating a patient, it is now possible to include numbers in the first or last name (e.g. for clinical studies
  • The settings (both on the device and in the web application) will now show the serial number of the device (as already shown on the rear side of the device.
